As an integral member of our nursing team, the Registered Nurse Planned Care Manager, Ambulatory Care will contribute to the strength of our team approach to collaborative medicine. You will also help to further our commitment to continuous improvement and our philosophy of empowering you to facilitate real change in the delivery of care.

Responsibilities of the Registered Nurse Planned Care Manager, Ambulatory Care include:

  • Managing patient populations with chronic illness and/or multiple co-morbidities.

  • Working in coordination with multiple PCPs and serves as the central link to an identified population of patients.

  • Providing skilled assessment, education, collaboration and coordination of healthcare and community resources,

  • Assisting patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life.

  • Assisting patients/families, staff and systems to achieve high quality, evidence-based, cost-effective, and patient-focused outcomes.

We require:

  • Current Washington state licensure as a Registered Nurse.

  • Bachelor's degree in Nursing (BSN)

  • Two to five years’ work experience in a Registered Nurse role.

  • Excellent patient assessment skills, ability to work collaboratively in team setting, current working knowledge of electronic medical records.

  • Experience managing complex patients in an ambulatory setting preferred.

  • Certification in diabetes education (CDE) preferred.
